We collect the information you provide on this form to process your expression of interest to provide content and contribute to The Transplant Network (such as participating in an interview with the network).

If we receive your expression of interest, we may contact you and discuss your journey with transplants and information about your health (including your medical conditions and your transplant journey). Providing this information is entirely voluntary. If you choose to participate in an interview with us, we will obtain your consent before we record and publish the information you provide.
